HIV Is 'Like Diabetes'? Let's Stop Kidding Ourselves

My antenna perked up when I read a gay man's comment that HIV infection is essentially no big deal anymore. "It's like diabetes these days," he said. As an HIV-positive 53-year-old, familiar with the health details of some near and dear diabetics, and having just written an article about the type-2 ("adult onset") diabetes epidemic, I'd say this: we need to choose our analogies carefully.

A naïve image of type-2 diabetes is understandable if your perception is based on the trim, middle-age joggers touting their finger-stick tests and blood glucose meters in TV commercials during the evening news. In actual fact, diabetes is the seventh leading cause of death in America. As many as 7 in 10 people with diabetes will die of heart disease or stroke. In 2007 diabetes killed or contributed to the deaths of 231,404 deaths in this country alone, and this year the World Health Organization projects 3.2 million worldwide will die from diabetes. Diabetes is also the leading cause of blindness. One of every 50 people with diabetes will be blind within 15 years. It's the top cause of kidney failure. In 2008 a total of 202,290 people with end-stage diabetes-related kidney disease depended on dialysis or a kidney transplant to stay alive. Diabetes is behind most non-traumatic lower-limb amputations. Thirteen million men in the U.S. over age 20 -- 11.8 percent of all adult men -- have diabetes. Among women, 12.6 million, or 10.8 percent of all women over age 20 have it. An estimated 7 million Americans are believed to have diabetes but don't know it. The percentages are highest among blacks, Hispanics and Asian-Americans.

Sure it's possible to manage type-2 diabetes once someone has a diagnosis and is aware that their pancreas isn't producing enough insulin to regulate their blood sugar. It's even better to prevent diabetes by reducing the behaviors that lead to obesity, the number-one cause of type-2 diabetes. At best, someone with type-2 diabetes can commit to a strict regimen of healthy eating and exercise that for some has reversed the disease to the point that they need no more medication. The American Diabetes Association says that five years of normal blood glucose readings without medication can be considered a "complete remission." Many more, however, struggle to alter their longtime eating habits or make time for exercise as basic as walking. Without making the changes it takes to successfully manage diabetes, they pit their medication against their poor nutrition and sedentary habits and hope for the best. They may be diligent about taking their meds and keeping their many medical appointments, but even the labels on the diabetes medications explain that they are to be "used along with diet and exercise," and, on some meds, "weight loss."

We need to banish the notion that HIV infection today is "like diabetes," in spite of their similarities. Consider:

Both are transmitted through intimate behavior, one through sex, and the other, frequently, through family habits passed down over generations. Both diseases are alike in that they are best avoided and challenging to manage. They both cost a great deal of money for medications, medical specialists, and lab work. Certainly, HIV and diabetes each could destroy your health and likely kill you if they aren't properly managed. As for people with type-2 diabetes seeking to manage their illness, a healthy diet and exercise strengthen an HIV-positive person's ability to handle the daily impact of toxic chemotherapies; the hassle of medical appointments and blood work every few months; the discipline of taking pills every day, and dealing with their physical side effects; and the emotional, financial, and psychological tolls of having a financially and socially expensive medical condition.

But beyond this, and in spite of the obvious differences between a viral disease and a metabolic one, the most striking difference between having HIV and type-2 diabetes today is this: There's not even a remote chance that changing my diet or exercise habits can cure what I have.

Isn't this a double standard?
  • End DOMA (Defense of Marriage Act). Prevents Federal employees and Gays in the Military from gaining equal benefits. Prevents Gay Partners from gaining equal benefits.
  • Pass a LGBT friendly ENDA (Employee Non-Discrimination Act). ENDA would prevent employment discrimination of LGBT workforce.
  • Boycott Salvation Army as they will not hire Gays! "DO NOT DONATE TO THEM."
  • BOYCOTT "Chick-fil-a" for donating money to anti-gay groups.
  • BOYCOTT KOCH INDUSTRIES PRODUCTS- to include their products: Angel Soft toilet paper / Brawny paper towels / Dixie plates, bowls, napkins & cups / Mardi Gras napkins and towels / Quilted Northern toilet paper / Soft 'n Gentle toilet paper / Sparkle napkins / Vanity fair napkins / Zee napkins / Georgia-Pacific paper products & envelopes / All Georgia-Pacific lumber & building products (INVISTA Products) / Lycra / Stainmaster Carpet. The billionaire - Koch brothers are ultra republican - tea bag founders/supporters that do not support causes of the LGBT community.
  • Boycott WALMART - The National Gay and Lesbian Task Force criticized Wal-Mart for denying employee benefits to same-s-x partners and for failing to prohibit discrimination based on gender identity. By comparison, the group said two other chain stores with a strong presence in New York, Costco and Walgreens, did much better.
  • "Tune Out Trump", Boycott all Trump Hotel, casinos, holdings, The Apprentice and NBC Network! At CPAC, Trump said he is considering a run for president as a Republican. In a interview with the Des Moines Register, Trump added that he opposes all forms of legal recognition for gay couples, not just marriage. "They should not be able to marry," he said. So why does NBC keep the Apprentice when Donald Trump has stated he may run for president? After making racist remarks about Obama and recv'g public backlash, Trump decided not to run for president after NBC renewed his awful TV Show. How can NBC re-new the clown's show after he pulled such hateful shenanigans?

Washington state Senate approves gay marriage bill

The Washington state Senate passed legislation on Wednesday night to legalize gay marriage, moving the state closer to joining six other U.S. states and Washington, D.C. in allowing same-sex nuptials.

The 28-21 vote in favor of the measure - three more votes than needed for passage - sent it on to the state's House of Representatives, where the bill is expected to win swift approval by a comfortable margin as early as next week.

Both legislative bodies are controlled by Democrats. But the political dynamic on the issue shifted after Governor Christine Gregoire, a Democrat in her final year in office, announced last month she would support the gay marriage bill.

The vote was greeted with raucous cheers from gay couples and their supporters, and with determined looks from opponents. People from both sides of the issue packed the Senate's public galleries to witness the vote.

With passage in the House seen as virtually assured, foes of same-sex matrimony say they will seek the measure's repeal with a referendum asking voters to reaffirm marriage as being exclusively between one man and one woman.

To qualify for the November ballot, they would need to collect at least 120,577 signatures of registered voters by July 6.

Six other states recognize same-sex marriage - New York, Massachusetts, Connecticut, Vermont, New Hampshire and Iowa - as does the District of Columbia. Supporters are moving to pass similar statutes in Maryland and New Jersey.

A referendum to legalize gay marriage in Maine has qualified for the November ballot there.

The stage was set for Wednesday's action after Washington state senate sponsors of the bill said last week they had secured the 25-vote minimum needed in the 49-seat chamber for approval there. Four Republicans ended up joining 24 Democrats in supporting it.

An emotional written statement released shortly before the Senate convened on Wednesday from one of the bill's 11th-hour supporters, Democratic Senator Brian Hatfield, expressed the personal conflict he and other lawmakers felt in backing the measure, which he said contradicted his Christian beliefs.

"Regardless of how I choose to vote on the issue of marriage equality, I will alienate myself from friends and neighbors that I have known for years," he wrote.

"A vote in favor of marriage equality will enrage those who see it as a stone cast against God and the beliefs that I and thousands like me have been raised with. A vote against will label me as a bigot who is against extending the basic rights that I enjoy to all residents of our state," he added.

The bill's chief Senate sponsor, Democrat Ed Murray, called the 28 votes "a pleasant surprise."

"I'm sincerely moved by my colleagues' courage, no matter how they voted," he added.

Murray, who is gay, told colleagues on the floor before the vote that he looked forward to marrying his partner of more than 20 years, and told all of them to expect an invitation in the mail no matter how they voted.

FASTER THAN EXPECTED

Washington state's lower House could vote on gay marriage as early as February 8. Should it reach the governor's desk, Gregoire plans to sign the measure, barring any drastic changes made during the rest of the legislative session, which ends March 8, a representative of the governor said.

Both House and Senate versions of the bill include language to allow gay couples from out of state to get married in Washington. They also both include an amendment allowing religious organizations to refuse to rent out their chapels or other facilities as venues for same-sex weddings.

The legislation was moving more quickly than originally anticipated, said Representative Jamie Pedersen, the bill's chief sponsor in the House.

"The groundswell of support ... was much better than what we could have hoped for," Pedersen said. "Big controversial things usually get resolved in the last days of session, so this is somewhat unusual."

Opponents point out that voters have yet to approve gay marriage in any state referendum and have in some instances overturned same-sex marriage laws enacted by legislators or the courts. Gay marriage remains outlawed in more than 40 of the 50 U.S. states.

Oregon voters passed a law upholding traditional marriage in 2004, voiding same-sex weddings that some counties had performed earlier that year. California voters in 2008 approved a state constitutional amendment banning gay marriage. That measure is under challenge in federal court.

Several prominent Washington-based companies employing tens of thousands of workers, including Microsoft, Amazon and Starbucks, have endorsed Washington's legislation.

On the other side, the state's Roman Catholic bishops and other religious conservatives are amassing thousands of Washington residents against the legislation, including voters who supported the state's expansion of domestic-partnership benefits in recent years but draw the line at gay marriage.
