Antiviral drugs stop HIV spreading to sexual partners

In a decisive breakthrough against the spread of HIV, an international study has demonstrated conclusively that antiretroviral therapy blocks the spread of HIV from an infected person to their uninfected partner.

The study, which was halted early because the results were so compelling, covered 13 countries and involved 1763 "discordant" couples, in which one partner carried the infection at the outset.

In 886 of the couples, the infected partner received antiretroviral drug therapy (ART) straight away, while treatment was delayed in the other 877 couples until the infected partner showed pre-defined signs of sickness.

Preliminary results, announced yesterday by the US National Institute of Allergy and Infectious Diseases (NIAID), showed that only one of the 886 individuals who received immediate treatment passed the virus to their partner. There were 27 cases of cross-infection in the 877 couples where treatment was delayed.

Faced with this 96 per cent reduction in risk, the study was halted between three and four years early so all of the couples could be treated.

HIV transmission

"This new finding convincingly demonstrates that treating the infected individual, and doing so sooner rather than later, can have a major impact on reducing HIV transmission," says Anthony Fauci, director of NIAID.

"This is a crucial development, because we know that sexual transmission accounts for about 80 per cent of all new infections," says Margaret Chan, director-general of the World Health Organization.

Called HPTN 052, the trial was led by Myron Cohen of the University of North Carolina at Chapel Hill. The results show that the treatment blocked transmission from either male or female partners, as there were equal numbers of both in the trial. Few gay couples were involved – just 3 per cent of those participating – so no knows yet whether the treatment would be equally effective for them.

All couples were counselled to continue their usual safe-sex practices, including use of condoms, to exclude this factor as a potential difference between the two groups.

To qualify for the trial, infected participants had to be at a relatively early stage of infection where CD4+ cells – the white blood cells targeted and depleted by HIV – were still moderately high, with counts of between 350 and 550 per millilitre of blood.

Developing countries

Current aid programmes aimed at spreading treatment to all developing countries typically wouldn't treat patients with CD4 counts this high. The count needs to fall below 350 before treatment begins – a strategy introduced by the WHO in 2009. But Cohen and his colleagues wanted to evaluate the potential for giving the drugs preventively in relatively healthy individuals, to see if it would help them avoid infecting their partners.

In the wake of the trial, the WHO will be under pressure to allow infected people with counts between 350 and 500 to qualify for treatment as part of a programme specifically aimed at preventing infection of partners.

The WHO will issue its new guidelines in July, and Chan said yesterday that "the findings from this study will further strengthen and support the new guidance that WHO is releasing in July to help people living with HIV protect their partners".

"We very strongly feel that treatment for prevention in this group must be an option they can choose," says Bernhard Schwartlander, director of evidence, strategy and results at UNAIDS.

"We know that ART very effectively suppresses the multiplication and level of virus in blood and mucosal cells, such as those lining the vagina, and we've already successfully applied this same principle for years in treatment to block mother-to-child transmission of HIV," says Schwartlander. "So if the prevention option is included in the guidelines in July, I think it would be a major step forward," he says.

The trial is the second this week to provide new hope against HIV, which has so far killed 30 million people with 33 million infections around the world. Earlier this week, a vaccine trial in monkeys produced the first conclusive evidence that a vaccine could completely suppress the virus, but it could take a decade to demonstrate whether the approach works in people.

  • Boycott Arizona the home of  "hater" - U.S. Senator John McCain. Arizona's Hateful anti-Immigration Law - encourages racial profiling and increases hatred towards minorities.  A remedy: Demand Republicans to  support Immigration Reform (The Dream Act). Further, Senator McCain voted against ending DADT and Blocked Immigration reform! 
  • Boycott Target, Best Buy, Gold's Gym - for donating money towards anti-gay political candidates/organizations. Update: (12/26/2010): Target is continuing to donate to anti-gay groups/causes/politicians.  Update 03/08/11 - Lady Gaga Ends Target Partnership, Reportedly Over Target's Poor LGBT Stance. Update 3/25/2010 - Target displays their hypocrisy and Sues California Gay Rights Group for Lobbying Outside Stores.  Target is attempting to block the LGBT right of free speech!  STOP SHOPPING AT TARGET!!!!!!!!!!!!!!!!!  Update 4/8/2011: Target lost it's case to stop the LGBT from canvassing in front of their stores.  A huge PR Disaster for TARGET!  Perhaps Target should go out of business as they betrayed their investers and customer base. Target contributed massive funds to a politician who would like to exterminate gays.  Now, Target can not stop us from Boycotting them, nor picketing, nor talking to customers near their stores; although,  Target allows a anti-gay company such as the Salvation Army to stand outside their door and speak with customers and raise money. Isn't this a double standard?  
  • End DOMA (Defense of Marriage Act).  Prevents Federal employees and Gays in the Military from gaining equal benefits. Prevents Gay Partners from gaining equal benefits.    
  • Pass ENDA (Employee Non-Discrimination Act).  ENDA would prevent employment discrimination of  LGBT workforce.
  • Boycott Salvation Army as they will not hire Gays! "DO NOT DONATE TO THEM."
  • BOYCOTT "Chick-fil-a" for donating to anti-gay marriage groups.
  • BOYCOTT KOCH INDUSTRIES PRODUCTS- to include their products: Angel Soft toilet paper / Brawny paper towels / Dixie plates, bowls, napkins & cups / Mardi Gras napkins and towels / Quilted Northern toilet paper / Soft 'n Gentle toilet paper / Sparkle napkins / Vanity fair napkins / Zee napkins / Georgia-Pacific paper products & envelopes / All Georgia-Pacific lumber & building products (INVISTA Products) / Lycra / Stainmaster Carpet.   The billionaire - Koch brothers are ultra republican - tea bag founders/supporters that do not support causes of the LGBT community. 
  • Boycott WALMART - The National Gay and Lesbian Task Force, force criticized Wal-Mart for denying employee benefits to same-s-x partners and for failing to prohibit discrimination based on gender identity. By comparison, the group said two other chain stores with a strong presence in New York, Costco and Walgreens, did much better.
  • Remember David Kato, Gay activist - Uganda, who was slain because he is gay!
  • NEW: "Tune Out Trump", Boycott all Trump Hotel, casinos, holdings, The Apprentice and NBC Network! At CPAC, Trump said he is considering a run for president as a Republican. In a interview with the Des Moines Register, Trump added that he opposes all forms of legal recognition for gay couples, not just marriage. "They should not be able to marry," he said. So why does NBC keep the Apprentice when Donald Trump has stated he may run for president? Boycott NBC, CNBC! Boycott advertisers of The Apprentice and Trump's other business's. Don't shop at any bullding with Donald Trump's name to include Trump Hotels, Casinos, Condo's, Offices, Shops!  The Donald has expressed extreme Hatred towards gays and now blacks with his birther remarks and anti-gay rhetoric.  Remember how poorly Trump  treated Rosie O'Donnel?  Now we know why he did that!  It was hate against a lesbian as we now know he is bigot.  Unforgiveable!  Dump NBC because they air Donald Trump's "The Apprentice"!  Start Watching CBS Evening News with Katie Couric (who is unfortunately leaving).
  • NEW: Boycott "APPLE." For the current owners and the aspiring owners of the I-phone,  Apple allows an application, "The Gay App", which is supposed to help gay individuals turn straight.  This is a step in the wrong direction.
  • Apple needs to know that the decision to allow this App on their products is a fallacious idea! Update: March 23, 2011:  Apple removed this app from USA stores; but, rumored to have them remain in some foreign stores.  This is offensive not just towards the LGBT community in the USA; but, offensive to  the entire LGBT community around the world.  How does anyone approve such a hateful, despicable app towards the LGBT community? Especially when Gay Teens are killing themselves after being harrassed by their church, parents, as well as  by bully's and teachers at school. The idea that this app was used, solicited is disgusting.

Gay Students in Canada Get Bullied, Too. A Lot, Actually.

Nearly two out of three non-heterosexual students feel unsafe in school, according to a national study by the University of Winnipeg.

The study, which included 3,700 Canadian high school students surveyed between December 2007 and June 2009, tried to identify how students experienced homophobic and transphobic incidents, including what happened, the impact of those experiences, and what the schools did afterwards. The findings generally suggest that high school sucks.

Homophobic comments - from other students and, more disturbingly, from teachers - were reported as a common and accepted part of going to school.

Key findings include:

+ Girls are more likely than boys to experience verbal and physical harassment due to their sexual orientation.

+ Aboriginal students are most likely to know an out student. They are also most likely to see classroom discussions of sexuality as positive.

+ All non-Aboriginal visible minorities are least likely to know an out student and least likely to report seeing classroom discussions of sexuality as positive.

+ 70% of all participating students reported hearing expressions like "that's so gay" in school on a daily basis.

+ 47% reported hearing "faggot," "lezbo," and "dyke" in school on a daily basis.

+ 58% of straight students find homophobic comments upsetting.

+64% of LGBTQ students feel unsafe at school, particularly in washrooms and gym changing rooms.

+ 21% of LGBTQ students have been physically harassed or assaulted in school.

+ 10% of straight students have been harassed or bullied by people who thought they were gay.

+ Students in schools with gay-straight alliances are more likely to be open about their sexual orientation.

How does this compare to the 2009 National School Climate Survey conducted in America by GLSEN?

+ 72.4% heard homophobic remarks, such as "faggot" or "dyke," frequently or often at school.

+ 61.1% of students reported that they felt unsafe in school because of their sexual orientation, and 39.9% felt unsafe because of their gender expression.

+ 40.1% of LGBTQ students had been physically harassed or assaulted in school for their sexual orientation, 27.2% harassed/assaulted for their gender presentation.

+ "Having a Gay-Straight Alliance in school was related to more positive experiences for LGBT students, including: hearing fewer homophobic remarks, less victimization because of sexual orientation and gender expression, less absenteeism because of safety concerns and a greater sense of belonging to the school community."

There's a lot said about the "trickle-down" theory - that if we take discrimination out of the US constitution, that will send a better message to kids about how to be nice to homos. This seems to be true in some regard, but the relentless desire of kids to be assholes to other kids is often hard to overcome.

The Canadian report also makes several recommendations, such as developing provincial and school-board-based anti-homophobia programs and safe school policies, giving teachers professional development programs related to sexual orientation issues, and encouraging schools to start gay-straight alliances, either through student initiatives or, if no students are willing to do it, through teachers.

All of which makes the Burnaby parents protesting their school district's new anti-discrimination policies even more ludicrous. According to the Globe and Mail, one supporter said, "f you changed everything in the original policy to say Chinese or black of Hispanic or any racial thing, it would be appalling to oppose it."

Of course, if it was appalling to oppose it before Egale released the finding of the study, it's even more appalling now.
