Does Botox help neck pain?

NEW YORK (Reuters Health) - The ingredient in a Botox injection probably won't help ease neck pain, nor will it help neck pain sufferers do physical activities any better or improve their quality of life, a new review of past studies suggests.

"The available evidence just suggests that it doesn't work," study author Dr. Paul Michael Peloso told Reuters Health. If patients have neck pain, "they and their physicians should be discussing other therapies," Peloso added.

The ingredient in Botox, called botulinum toxin, is used to treat a range of conditions, including wrinkles, migraines, and excessive sweating. Botox is just one of several brand names for the drug, which is given through injections. None of the brands has been approved by the Food and Drug Administration to treat neck pain -- but doctors may use the drug "off-label" for that purpose.

A recent report from the Institute of Medicine suggested that chronic pain, including neck pain, costs the United States more than $600 billion each year and affects about four in 10 adults (see Reuters story of June 29, 2011).

Peloso, the head of clinical research at the drug company Merck in Rahway, New Jersey, said doctors might consider using the Botox ingredient for neck pain after other more common treatments, including drugs like Tylenol and aspirin or exercises, haven't worked.

Each injection typically costs a few hundred dollars, and patients may need repeat injections every few months for the drug to have a continued effect.

The new review analyzed the combined results of nine studies that tested botulinum toxin A -- sold as Botox by Allergan -- in people with new or chronic neck pain and related headaches.

Those studies compared the effect of botulinum toxin with drug-free sham injections or other pain treatments, using patient reports or doctor observations of pain, physical abilities, and quality of life. Altogether the studies involved 503 patients, including 272 who received injections of the drug.

Peloso and his colleagues noted that some of the studies had limitations -- for example, it wasn't clear how some had split the patients up into Botox and not-Botox groups, or researchers hadn't kept treatment types hidden from patients or from those who measured their symptoms.

But taken together, the studies suggested Botox had done nothing to help relieve neck pain or related headaches, either alone or when added to exercise programs or other medications, when researchers reassessed patients at four weeks and six months after injections.

In addition, people given botulinum toxin were more likely to have a side effect, including soreness or flu-like symptoms, than those not given the drug.

The researchers concluded that almost all of the studies showed that any possible benefits were not worth the potential harms. Their findings are presented in the Cochrane Library, a publication of the Cochrane Collaboration, an international organization that evaluates medical research.

A company spokesperson told Reuters Health in an email that "Allergan does not have clinical programs evaluating the efficacy and safety of our botulinum toxin products in patients with subacute or chronic neck pain," but that Botox is indicated to treat "cervical dystonia," in which neck muscles contract involuntarily, and neck pain linked to the condition. She also pointed out that the review didn't focus only on Botox-brand treatments.

Peloso said that while some evidence suggests injections of steroids or anesthetics might help people with neck pain, the best pain-relief choice right now seems to be exercises that strengthen muscles and improve range of motion.

However, he pointed out, "The problem is as you would guess for exercise that it's just not as simple as a one-time visit" or a daily drug -- one reason his team was interested to see if Botox might have some benefit.

He said that companies have little incentive to study treatments for neck pain, which can be caused by anything from overuse of neck muscles to diseases such as arthritis and gets more common with age. Peloso added that his company does not make a Botox competitor, although it does market pain products that could be used for neck pain.

  • Boycott Target, Best Buy, Gold's Gym - for donating money towards anti-gay political candidates/organizations. Update: (12/26/2010): Target is continuing to donate to anti-gay groups/causes/politicians. Update 03/08/11 - Lady Gaga Ends Target Partnership, Reportedly Over Target's Poor LGBT Stance. Update 3/25/2010 - Target displays their hypocrisy and Sues California Gay Rights Group for Lobbying Outside Stores. Target is attempting to block the LGBT right of free speech! STOP SHOPPING AT TARGET!!!!!!!!!!!!!!!!! Update 4/8/2011: Target lost its case to stop the LGBT from canvassing in front of their stores. A huge PR Disaster for TARGET! Perhaps Target should go out of business as they betrayed their investers and customers. Target contributed massive funds to a politician who would like to exterminate gays. Now, Target can not stop us from Boycotting them, nor picketing, nor talking to customers near their stores; although, Target allows a anti-gay company such as the Salvation Army to stand outside their door and speak with customers and raise money. Isn't this a double standard? Perhaps TARGET would better understand the issue if they cared about David Kato - Uganda, who was slain because he is gay! That is because Uganda wants to kill gay people. Again, TARGET invested in a political candidate that would Kill the Gays!
  • End DOMA (Defense of Marriage Act). Prevents Federal employees and Gays in the Military from gaining equal benefits. Prevents Gay Partners from gaining equal benefits.
  • Pass ENDA (Employee Non-Discrimination Act). ENDA would prevent employment discrimination of LGBT workforce.
  • Boycott Salvation Army as they will not hire Gays! "DO NOT DONATE TO THEM."
  • BOYCOTT "Chick-fil-a" for donating to anti-gay marriage groups.
  • BOYCOTT KOCH INDUSTRIES PRODUCTS- to include their products: Angel Soft toilet paper / Brawny paper towels / Dixie plates, bowls, napkins & cups / Mardi Gras napkins and towels / Quilted Northern toilet paper / Soft 'n Gentle toilet paper / Sparkle napkins / Vanity fair napkins / Zee napkins / Georgia-Pacific paper products & envelopes / All Georgia-Pacific lumber & building products (INVISTA Products) / Lycra / Stainmaster Carpet. The billionaire - Koch brothers are ultra republican - tea bag founders/supporters that do not support causes of the LGBT community.
  • Boycott WALMART - The National Gay and Lesbian Task Force, force criticized Wal-Mart for denying employee benefits to same-s-x partners and for failing to prohibit discrimination based on gender identity. By comparison, the group said two other chain stores with a strong presence in New York, Costco and Walgreens, did much better.

In a interview with the Des Moines Register, Trump added that he opposes all forms of legal recognition for gay couples, not just marriage. "They should not be able to marry," he said. So why does NBC keep the Apprentice when Donald Trump has stated he may run for president? After making racist remarks about Obama and recv'g public backlash, Trump decided not to run for president after NBC renewed his awful TV Show. How can NBC renew the clown's show after he pulled all such low-class shenanigans? Boycott NBC! Boycott advertisers of The Apprentice and Trump's other business's. Don't shop at any bullding with Donald Trump's name to include Trump Hotels, Casinos, Condo's, Offices, Shops! The Donald has expressed extreme Hatred towards gays and blacks with his birther remarks and anti-gay rhetoric. Remember how poorly Trump treated Rosie O'Donnel? Now we know why he did that! It was hate against a lesbian as we now know he is biget. Unforgiveable! Dump NBC because they re-newed Donald Trump's "The Apprentice"! NBC, Really? Indian Minister Clarifies Anti-Gay Remarks

India's health minister, Ghulam Nabi Azad, who told an AIDS gathering this week that sexual relations between men were "unnatural" and "a disease," on Tuesday clarified his remarks in a singularly unclear fashion.

"I have said something but half has been quoted, half has not been quoted," Mr. Azad told reporters. "We don't want controversy because we have had already enough in two years."

Mr. Azad, who appeared to be referring to his government's bad publicity over corruption allegations, also said that he never even used the word gay in his remarks.

"I am sorry if I have hurt feelings of any section of the society. During my speech I used technical words. I kept myself very clear, didn't use the word homosexual or gay," said Mr. Azad, according to a report on the website of news channel NDTV.  "The subject was HIV, [I] used man having sex with man in Hindi for contracting HIV."

That was not what it sounded like on Monday, when Mr. Azad spoke to a gathering in New Delhi of district council leaders from across the country.

"Unfortunately, this disease, where a man has sex with a man, which is absolutely unnatural and shouldn't happen but it is happening, is spreading around the world and has also come to India," said Mr. Azad, speaking in Hindi. "Even in our country the numbers of men having sex with men is significant."

Gay people and AIDS prevention activists said they were horrified, with some calling for his resignation. By the time Mr. Azad clarified his remarks, even the United Nations AIDS organization had weighed in.

"India's rich tradition of inclusivity and social justice must include men who have sex with men and transgender people," said Michel Sidibé, executive director of UNAIDS, according to a statement.  "There is no place for stigma and discrimination on the basis of sexual orientation."

Mr. Azad's remarks, like former South African president Thabo Mbeki's HIV skepticism, caused concern among gay rights and AIDS prevention groups that, in addition to being offensive to gay people, this would set back health efforts by showing that stigma around homosexuality exists even at the highest reaches of the Indian government.
