Over 100 000 South African young men were circumcised in the last financial year in the government's programme to integrate medical male circumcision into the country's HIV prevention strategy.
After what some might consider a slow start, the Department of Health's roll-out of medical male circumcision or MMC under the guidance of Health Minister, Dr Aaron Motsoaledi, has got a significant number of men to respond to medical circumcision to protect themselves against HIV infection. The Department says it surpassed its targets for the year 2010 - 2011.
"For the old financial year, 2010 - 2011, we have done 140 000 circumcisions. Some of the provinces have over-shot the target and some are moving towards the target. We have done well. We were given R40 million to do 100 000 circumcisions for the previous financial year", says programme manager for medical male circumcision, Dayanund Loykissoonlal.
Medical male circumcision is increasingly being integrated into national HIV prevention programmes, especially in settings where HIV prevalence is high and the prevalence of circumcision is low, such as in KwaZulu-Natal. Scientific research has shown that the practice reduces the risk of HIV infection for men by up to 60%. However, circumcised men still need to use other proven HIV preventative methods, such as condoms.
Loykissoonlal says the country plans to circumcise 80% of men aged 15 - 49 in the next five years. "We would like to do, at least, 5.7 million circumcisions over the next 5 years, including this year", he says.
With these aggressive targets, Government will need buy-in from key stake- holders such as the traditional sector to achieve their goals.
"Circumcision is part and parcel of the South African culture. If you have to move from province to province, you'll notice that a certain percentage of circumcisions are done. But this is traditional circumcision. We are very sensitive towards the traditional practice. We need to look at this seriously and integrate traditional circumcision into the current protocols", says Loykissoonlal.
Loykisoonlal adds that the Government is currently in consultation with traditional practitioners and their rural communities to get their buy-in and support.
"The minister asked us that we interact with traditional circumcisers. All of us understand that it's a major hurdle we need to cross. It's a kind of a consultative process for now", he says.
Thabisa Vile, a mother and a staunch supporter of traditional circumcision warned government to tread carefully when engaging with the traditional sector about medical male circumcision.
"When you are promoting MMC for it to kind of gain the momentum you want, don't criticise traditional circumcision. Rather come with ways of embracing it because I want my child to be circumcised traditionally, but in a healthy and safe manner.
As much as you are coming with this very good intervention, be very sensitive to our culture. It is one of the things that we have pride in and we want to hold on to. So, if ever there are HIV issues being addressed in traditional circumcision, then the best way is to address those issues with traditional circumcisers, instead of saying that medical male circumcision is the way to go", says Thabisa Vile.
Dr Dino Rech, the Medical Director of HIV and AIDS Prevention Studies (CHAPS), says a lot needs to be done to promote medical male circumcision to reach as many young men as possible.
"How do you mobilise boys to come for a service which causes pain and has to do with removing foreskin from one's genitals? It's probably the biggest social marketing challenge ever and getting that right is the first complex, difficult step", he says.
Dr. Rech also said that medical male circumcision sites need to be placed in areas where people can access them by foot, taxis and trains, as transport is an issue.
"One of the biggest barriers to boys accessing services is, often, transport" says Dr Rech.
He also added that it is important to ensure that every young man who undergoes the surgical procedure comes back for after-care.
"We don't cut the boys, get our numbers and forget about them. We need to make sure they are coming back and we need to record and monitor how they are coming back", concludes Dr Rech.

Update 3/25/2010 - Target displays their hypocrisy and Sues California Gay Rights Group for Lobbying Outside Stores. Target is attempting to block the LGBT right of free speech! STOP SHOPPING AT TARGET!!!!!!!!!!!!!!!!! Update 4/8/2011: Target lost it's case to stop the LGBT from canvassing in front of their stores. A huge PR Disaster for TARGET! Perhaps Target should go out of business as they betrayed their investers and customer base. Target contributed massive funds to a politician who would like to exterminate gays. Now, Target can not stop us from Boycotting them, nor picketing, nor talking to customers near their stores; although, Target allows a anti-gay company such as the Salvation Army to stand outside their door and speak with customers and raise money. Isn't this a double standard?
- End DOMA (Defense of Marriage Act). Prevents Federal employees and Gays in the Military from gaining equal benefits. Prevents Gay Partners from gaining equal benefits.
- Pass ENDA (Employee Non-Discrimination Act). ENDA would prevent employment discrimination of LGBT workforce.
- Boycott Salvation Army as they will not hire Gays! "DO NOT DONATE TO THEM."
- BOYCOTT "Chick-fil-a" for donating to anti-gay marriage groups.
- BOYCOTT KOCH INDUSTRIES PRODUCTS- to include their products: Angel Soft toilet paper / Brawny paper towels / Dixie plates, bowls, napkins & cups / Mardi Gras napkins and towels / Quilted Northern toilet paper / Soft 'n Gentle toilet paper / Sparkle napkins / Vanity fair napkins / Zee napkins / Georgia-Pacific paper products & envelopes / All Georgia-Pacific lumber & building products (INVISTA Products) / Lycra / Stainmaster Carpet. The billionaire - Koch brothers are ultra republican - tea bag founders/supporters that do not support causes of the LGBT community.
- Boycott WALMART - The National Gay and Lesbian Task Force, force criticized Wal-Mart for denying employee benefits to same-s-x partners and for failing to prohibit discrimination based on gender identity. By comparison, the group said two other chain stores with a strong presence in New York, Costco and Walgreens, did much better.

How can NBC re-new the clown's show after he pulled such hateful shenanigans? Boycott NBC! Boycott advertisers of The Apprentice and Trump's other business's. Don't shop at any bullding with Donald Trump's name to include Trump Hotels, Casinos, Condo's, Offices, Shops! The Donald has expressed extreme Hatred towards gays and blacks with his birther remarks and anti-gay rhetoric. Remember how poorly Trump treated Rosie O'Donnel? Now we know why he did that! It was hate against a lesbian as we now know he is biget. Unforgiveable! Again, Dump NBC because they air/re-newed Donald Trump's "The Apprentice"! Now the Donald Claims the Long Form Birth certificate is a fraud. And NBC keeps this guy?
