How To Talk To A Partner About HIV, AIDS and STDs

Q: I've gotten pretty confused about when to talk with new partners or boyfriends about STDs and HIV status. Some of my friends say that as long as you're having safer sex, there's no need to have "the talk." Others say -- vehemently, I might add -- that I must have that conversation. What do you think? By the way I'm HIV-positive.

: First of all, I can't help but note that this week marks the 30th year since the Centers for Disease Control and Prevention first identified the disease now known as AIDS. With more than 60 million infected worldwide and nearly half that number dead, it's important for us to remember that this epidemic continues, both here and overseas. It's also key to point out that the changes in sexual behavior initiated by the gay community back in the 80s are largely responsible for curbing the spread of HIV/AIDS in the U.S.

I asked a number of experts in the HIV/AIDS field about your question, because there hasn't always been a consensus on the finer points of disclosure. After talking to them and weighing all the information, here's my take: All of us who are sexually active need to be responsible for considering the risks of spreading STDs and for taking steps to protect ourselves and our partners. Whether positive or negative (or, God forbid, unknowing), we owe it to everyone involved to talk about our sexual health before having sex. I've long said that if you're intimate enough to have sex with someone, you're intimate enough to talk about HIV status.

Still, it's usually easier to do this before you find yourself in the heat of the moment, where folks sometimes get carried away by the throes of passion and take risks they might not otherwise. "The talk" needn't be involved or lengthy -- although, admittedly, it can be hard to make this particular topic romantic -- but this isn't a time to be reticent.

Be truthful and direct, saying, for instance: "I just want you to know that as far as I know, I'm [fill in the blank]. What about you?" Sometimes it's easier if you volunteer your health status first, as a way to open the door. If humor comes to you naturally, by all means try that; but remember, you're not giving a public health lecture.

Since you mentioned that you're HIV-positive, let me give you some more advice to chew on. Even if you've hinted at your seropositive status, don't assume your partner knows. The subtle signals of human interaction -- especially sex-charged interaction -- are easily misinterpreted. The words you'll choose will depend both on your personality and on the kind of connection you have with this person, but you should be clear no matter what. Some come right out and say, "I'm HIV-positive." Others get there sideways, by asking, for example: "Have you ever dated someone who is HIV-positive?" and then steering the talk toward their own health status.

Similarly, it's smart for HIV-negative people to tell their partners that information, too. This may well help a positive partner disclose his status or help both of you gauge where you'll play on the safer-sex spectrum.

All that said, there are some caveats. (Nothing is simple these days.) If you're planning to have anonymous sex, say at a club or bathhouse, it's hard to imagine having much of a conversation about anything, much less about HIV status. In this case, I'd say that you, as an HIV-positive man, must assume there are HIV-negative people present and that you should therefore practice safer sex all the time. Of course, everyone may define "safer sex" differently; for instance, where does fellatio without a condom fall on your personal spectrum? An HIV-negative person who finds himself at a similar venue should assume that HIV-positive people will be among his partners and should protect himself accordingly.

  • Boycott Bill O'Reilly and FOX News and their advertisers. Bill discussed a French commercial by McDonalds that was meant to show that McDonalds is Gay Friendly. Bill said what is next? Is McDonalds going to be Al-Qaida friendly as well? Imagine - Bill O'Reilly compares Gays to Al-Qaida! In conclusion, O'Reilly expressed out-right Hatrad of the LGBT Community.
  • Boycott Arizona the home of "hater" - U.S. Senator John McCain. Arizona's Hateful anti-Immigration Law - encourages racial profiling and increases hatred towards minorities. A remedy: Demand Republicans to support Immigration Reform (The Dream Act). Further, Senator McCain voted against ending DADT and Blocked Immigration reform!
  • Boycott Target, Best Buy, Gold's Gym - for donating money towards anti-gay political candidates/organizations. Update: (12/26/2010): Target is continuing to donate to anti-gay groups/causes/politicians. Update 03/08/11 - Lady Gaga Ends Target Partnership, Reportedly Over Target's Poor LGBT Stance. Update 3/25/2010 - Target displays their hypocrisy and Sues California Gay Rights Group for Lobbying Outside Stores. Target is attempting to block the LGBT right of free speech! STOP SHOPPING AT TARGET!!!!!!!!!!!!!!!!! Update 4/8/2011: Target lost it's case to stop the LGBT from canvassing in front of their stores. A huge PR Disaster for TARGET! Perhaps Target should go out of business as they betrayed their investers and customers. Target contributed massive funds to a politician who would like to exterminate gays. Now, Target can not stop us from Boycotting them, nor picketing, nor talking to customers near their stores; although, Target allows a anti-gay company such as the Salvation Army to stand outside their door and speak with customers and raise money. Isn't this a double standard? Perhaps TARGET would better understand the issue if they cared about David Kato - Uganda, who was slain because he is gay! That is because Uganda wants to kill gay people. Again, TARGET invested in a political candidate that would Kill the Gays!
  • End DOMA (Defense of Marriage Act). Prevents Federal employees and Gays in the Military from gaining equal benefits. Prevents Gay Partners from gaining equal benefits.
  • Pass ENDA (Employee Non-Discrimination Act). ENDA would prevent employment discrimination of LGBT workforce.
  • Boycott Salvation Army as they will not hire Gays! "DO NOT DONATE TO THEM."
  • BOYCOTT "Chick-fil-a" for donating to anti-gay marriage groups.
  • BOYCOTT KOCH INDUSTRIES PRODUCTS- to include their products: Angel Soft toilet paper / Brawny paper towels / Dixie plates, bowls, napkins & cups / Mardi Gras napkins and towels / Quilted Northern toilet paper / Soft 'n Gentle toilet paper / Sparkle napkins / Vanity fair napkins / Zee napkins / Georgia-Pacific paper products & envelopes / All Georgia-Pacific lumber & building products (INVISTA Products) / Lycra / Stainmaster Carpet. The billionaire - Koch brothers are ultra republican - tea bag founders/supporters that do not support causes of the LGBT community.
  • Boycott WALMART - The National Gay and Lesbian Task Force, force criticized Wal-Mart for denying employee benefits to same-s-x partners and for failing to prohibit discrimination based on gender identity. By comparison, the group said two other chain stores with a strong presence in New York, Costco and Walgreens, did much better.
  • NEW: "Tune Out Trump", Boycott all Trump Hotel, casinos, holdings, The Apprentice and NBC Network! At CPAC, Trump said he is considering a run for president as a Republican. In a interview with the Des Moines Register, Trump added that he opposes all forms of legal recognition for gay couples, not just marriage. "They should not be able to marry," he said. So why does NBC keep the Apprentice when Donald Trump has stated he may run for president? After making racist remarks about Obama and recv'g public backlash, Trump decided not to run for president after NBC renewed his awful TV Show. How can NBC renew the clown's show after he pulled all such low-class shenanigans? Boycott NBC! Boycott advertisers of The Apprentice and Trump's other business's. Don't shop at any bullding with Donald Trump's name to include Trump Hotels, Casinos, Condo's, Offices, Shops! The Donald has expressed extreme Hatred towards gays and blacks with his birther remarks and anti-gay rhetoric. Remember how poorly Trump treated Rosie O'Donnel? Now we know why he did that! It was hate against a lesbian as we now know he is biget. Unforgiveable! Dump NBC because they re-newed Donald Trump's "The Apprentice"! NBC, Really? Update June 1, 2011: Trump now claims that Obama's long form, original birth certificate is fake. And NBC keeps this hater? NBC, You Kidding Us?

Rates of suicide, self-harm and bullying are high in LGBT people. Repressing discussion of our sexuality will only worsen the problem.

Growing up being different is not easy. This is especially true in schools: signs of weakness or difference attract bullies like ugly moths to the light. This is especially true for LGBT students, who are statistically more likely to be bullied.

I was unlucky enough to be one of these lights, and a very attractive one at that - I wear glasses when I'm working, I'm a geek, I have a funny accent and I'm attracted to other women. I've left that treatment behind permanently, but I'm all too aware that there are other students who still have to endure this abuse. I've even encountered some shocking homophobia in conversations with older people. I won't dissect the sentiments in detail, but I will say that it involved aliens and abandoning a certain demographic on their planet.

There are public figures who continue to lend an air of legitimacy to homophobic attitudes and indirectly condone the bullies. Peter Heck, a conservative US radio host and columnist, recently claimed that coming out as lesbian, gay or bi was just "an alarming fad" and that these types of sexuality are "decadent". The straw man that gay people aim to "incessantly shove their unconventional behavior in front of our children's faces" also made an appearance - emotional blackmail involving children is often used as a substitute for logical argument in debates about sexuality.

Heck goes on to say that "there is absolutely no courage to be found" in someone declaring that they are gay, lesbian, bi or transsexual. Well, you know what, Peter? When you've gone through years of abuse and harassment , I can guarantee that you'd find coming out of the closet a frightening experience too. The feeling of acceptance can be wonderful, but rejection can be deeply wounding. And, sadly, rejection is not uncommon in a culture where organisations such as Narth in the US continue to encourage patients to "develop their heterosexual potential".

Young LGBT people face a higher rate of self-harm and suicide than their heterosexual peers. This unfortunate pattern is also seen in older LGBT groups as well - a study by the Brighton & Hove LGBT Suicide Prevention Working Party shows that people in this group who are under 40 are three times as likely to have considered suicide as a similar heterosexual group, and more likely to turn to substance abuse.

Some representatives of conservative ideologies are keen to take a hardline stance and further supress any deviation from expected gender norms. Peter Sprigg of the Family Research Council in the US proposes the following solution to the high LGBT suicide rate: "The most effective way of reducing teen suicide attempts is not to create a 'positive social environment' for the affirmation of homosexuality. Instead, it would be to discourage teens from self-identifying as gay, lesbian, or bisexual." This is actually a compromise on his desired aims - he'd rather see homosexual acts banned than simply repress self-identification of sexuality.

This repression is likely to be counter-productive - data is not the plural of anecdote - but hiding a large part of your identity can cause a lot of stress, especially when it comes to romance and relationships. The idea that being LGBT is something that should be hidden only gives further credence to the idea that these traits are shameful and clandestine - after all, talking about the day out you had with your same-sex partner is hardly material fit for casual conversation, is it?

Sprigg says explicitly that he doesn't think creating a positive social environment for LGBT youth will help the problem of high suicide and self-injury rates, despite the evidence that a supportive environment is thought to reduce the chances of suicide attempts in this group. The Tennessee senate appears to agree with him on this one, introducing the "Don't say gay" bill earlier last month. While the UK's section 28 banned promoting homosexuality or "acceptability of homosexuality as a pretended family relationship", the new Tennessee bill calls for instruction and materials used in schools to be "limited exclusively to age-appropriate natural human reproduction science". This excludes all education about same-sex relationships and anything else deemed "unnatural" by the powers that be.

However, I'm delighted to announce that creative souls have already been at work on undermining this example of homophobia. The Star Trek actor George Takei has been kind enough to lend his name to the cause, encouraging people affected by this bill to publicly support Takei rights and Takei marriage. And unless the authorities start claiming that George Takei is unnatural, there is nothing the bill can do to prevent this.

Although people like Sprigg may genuinely be trying to offer a solution to these awful statistics, they are only contributing to the problems that cause them in the first place. We need an antidote to these attitudes - while they are given a veneer of respectability, LGBT youth and adults alike will continue to suffer for a trait we did not ask for or choose. It's not our sexuality that is causing us harm - it's the bigotry surrounding us.
