HIV infection is a risk factor for heart failure (HF), with ongoing viral replication associated with higher risk, according to the results of a population-based, retrospective study reported in the April 25 issue of the Archives of Internal Medicine.
"Whether human immunodeficiency virus (HIV) infection is a risk factor for heart failure (HF) is not clear," according to previous research as cited in the current study. "The presence of coronary heart disease [CHD] and alcohol consumption in this population may confound this association."
In the current study, Adeel A. Butt, MD, MS, from the University of Pittsburgh School of Medicine, Pittsburgh, and Pittsburgh Healthcare System, Pennsylvania, and colleagues investigate this possible association between HIV and HF.
Their study population consisted of 8486 veterans, of whom 28.2% were HIV infected, enrolled in the Veterans Aging Cohort Study Virtual Cohort (VACS-VC) and the 1999 Large Health Study of Veteran Enrollees (LHS) from January 1, 2000, to July 31, 2007. There were 286 incident HF events during follow-up (median duration, 7.3 years).
Among HIV-infected veterans, age- and race/ethnicityadjusted rates of HF were 7.12 (95% confidence interval [CI], 6.90 - 7.34) per 1000 person-years vs 4.82 (95% CI, 4.72 - 4.91) per 1000 person-years among HIV-uninfected veterans. Risk for HF was increased in HIV-infected vs HIV-uninfected veterans (adjusted hazard ratio [HR], 1.81; 95% CI, 1.39 - 2.36), even among those without a CHD event or a diagnosis related to alcohol abuse or dependence before the incident HF event (adjusted HR, 1.96; 95% CI, 1.29 - 2.98).
The risk for HF was higher in HIV-infected veterans with a baseline HIV-1 RNA level of at least 500 copies/mL vs HIV-uninfected veterans (adjusted HR, 2.28; 95% CI, 1.57 - 3.32). However, the risk for HF in HIV-infected veterans with baseline and recent HIV-1 RNA levels of less than 500 copies/mL was comparable to that in HIV-uninfected veterans (adjusted HR, 1.10; 95% CI, 0.64 - 1.89; P < .001 for groups with high vs low HIV-1 RNA levels).
"Our data suggest that HIV infection is a risk factor for HF," the study authors write. "Ongoing viral replication is associated with a higher risk of developing HF."
Limitations of this study include inability to exclude subclinical CHD, diagnosis of HF based on International Classification of Diseases, Ninth Revision, codes, inability to assess the effect of specific antiretroviral drugs or adherence to medication therapy, lack of generalizability to women, and inability to distinguish between HF associated with systolic vs diastolic dysfunction.
"If HF is a major cardiovascular consequence of HIV infection rather than atherosclerotic heart disease, different approaches to manage such consequences are warranted," the study authors conclude. "Cardiovascular risk factor reduction and antiplatelet agents (eg, aspirin) are the mainstay in the management of atherosclerotic heart disease; however, these strategies, plus aggressive blood pressure control and the treatment of the HIV infection, may also be required to prevent development of HF in this population. The exact approach would depend on the mechanism and mediators of this risk; therefore, further studies are needed."

STOP SHOPPING AT TARGET!!!!!!!!!!!!!!!!! Update 4/8/2011: Target lost it's case to stop the LGBT from canvassing in front of their stores. A huge PR Disaster for TARGET! Perhaps Target should go out of business as they betrayed their investers and customer base. Target contributed massive funds to a politician who would like to exterminate gays. Now, Target can not stop us from Boycotting them, nor picketing, nor talking to customers near their stores; although, Target allows a anti-gay company such as the Salvation Army to stand outside their door and speak with customers and raise money. Isn't this a double standard?

- BOYCOTT KOCH INDUSTRIES PRODUCTS- to include their products: Angel Soft toilet paper / Brawny paper towels / Dixie plates, bowls, napkins & cups / Mardi Gras napkins and towels / Quilted Northern toilet paper / Soft 'n Gentle toilet paper / Sparkle napkins / Vanity fair napkins / Zee napkins / Georgia-Pacific paper products & envelopes / All Georgia-Pacific lumber & building products (INVISTA Products) / Lycra / Stainmaster Carpet. The billionaire - Koch brothers are ultra republican - tea bag founders/supporters that do not support causes of the LGBT community.
- Boycott WALMART - The National Gay and Lesbian Task Force, force criticized Wal-Mart for denying employee benefits to same-s-x partners and for failing to prohibit discrimination based on gender identity. By comparison, the group said two other chain stores with a strong presence in New York, Costco and Walgreens, did much better.

The Donald has expressed extreme Hatred towards gays and now blacks with his birther remarks and anti-gay rhetoric. Remember how poorly Trump treated Rosie O'Donnel? Now we know why he did that! It was hate against a lesbian as we now know he is bigot. Unforgiveable! Dump NBC because they air Donald Trump's "The Apprentice"! Start Watching CBS Evening News with Katie Couric (who is unfortunately leaving).

Apple needs to know that the decision to allow this App on their products is a fallacious idea! This is offensive not just towards the LGBT community in the USA; but, offensive to the entire LGBT community around the world. How does anyone approve such a hateful, despicable app towards the LGBT community? Especially when Gay Teens are killing themselves after being harrassed by their church, parents, as well as by bully's and teachers at school. The idea that this app was used, solicited is disgusting.

I recently had the great pleasure of seeing a preview of the new Broadway revival of Larry Kramer's challenging and controversial play, The Normal Heart, which opened to the public this week. The Normal Heart tells the harrowing and inspirational true story of a group of gay men in New York City who come together in the early 1980s to found the organization, Gay Men's Health Crisis, and fought largely unsympathetic political and media powers for funding and attention to address the burgeoning AIDS epidemic in America.
Back in 1985, Larry's extraordinary play tackled the difficult topic of AIDS when very few people were willing to speak out about an escalating public health crisis. Scientists were still searching for effective treatments, AIDS wards were full of dying people, and many politicians were more interested in blaming the sick for their illness than in finding solutions and funding preventions. The play helped to galvanize the gay community, inspiring many capable people to step forward and become a new generation of activists and community leaders.
So, what do we have to learn from a nearly 30-year-old play about the earliest days of the AIDS epidemic? Fortunately, by giving us a window back into those dark times, Larry's play shows us how far we have come in many aspects of addressing this now global epidemic. Thanks to artists like Larry, advocates like Sir Elton John, and the late Dame Elizabeth Taylor, and countless people in and out of power who chose to step forward and speak out, we have made great strides in terms of the public perception of HIV/AIDS. Scientific research has produced an arsenal of lifesaving treatments helping many people live longer, healthier lives despite their HIV infection. Organizations like the Elton John AIDS Foundation and its grantees are helping to bring the latest in HIV prevention methods, education, and treatments into under-served communities across the U.S. and the developing world.
However, while it is true that a great deal of progress has been made, it is nevertheless tremendously shocking just how much hasn't changed -- making The Normal Heart as timely today as it was in 1985. In fact, Larry Kramer was very much ahead of his time in artfully communicating the most insidious aspects of the epidemic. Many of the issues presented in the play remain hot button topics today. For instance, in addition to the AIDS crisis, the play also tackles the issues of same sex marriage and health care reform.
More importantly, even after more than 25 years of HIV prevention and public education efforts, stigma and prejudice against people living with HIV/AIDS still prevent progress in reducing the incidence of HIV infection. The Elton John AIDS Foundation and other organizations devoted to HIV prevention share a deep frustration over the mounting numbers of new infections -- around 56,000 in the U.S. every year -- especially when polls show that fewer and fewer Americans regard HIV/AIDS as a significant public health concern. Just like the early years of the epidemic, complacency and a lack of urgency about HIV/AIDS, both in the media and in the general public, are quietly and literally killing us.
It is vital that younger generations understand the history of the HIV/AIDS movement -- how dire things really were in the early 1980s, how progress was made, and what remains to be done. "The Normal Heart" dramatizes history in a way that creates a visceral connection to the early days of the AIDS epidemic in America. It is sure to inspire today's youth to get involved just as it did 25 years ago.
Aside from being a beautiful performance of a milestone play, this new production of The Normal Heart is a potent reminder of where we've been and where we need to go in the fight against AIDS. This is achieved with the help of a truly extraordinary cast. Ellen Barkin brings all of the magnetism, commitment, and courage she has displayed in her film roles to the Broadway stage. Joe Mantello gives a performance that transports the audience to another time and place. Under the co-direction of George C. Wolfe and Joel Grey, this production powerfully connects theatergoers to the boiling cauldron of issues that emerged at the onset of the AIDS crisis and that continue to challenge us today.
On behalf of all those who are deeply committed to reducing stigma and reversing the AIDS epidemic in the US and around the world, I am profoundly grateful to Larry Kramer and the producers of The Normal Heart for bringing this important work back to the stage. I encourage everyone to see and support this courageous play.
Every gay and lesbian person who has been lucky enough to survive the turmoil of growing up is a survivor. Survivors always have an obligation to those who will face the same challenges."
