Simple Rules About Aortic Disease

By Linda Childers, April 26, 2011

After actor John Ritter died from an aortic dissection – a tear in an artery leading from the heart – his wife, Amy Yasbeck, dedicated herself to educating others about this often-misdiagnosed medical condition. In an exclusive Lifescript interview, Yasbeck discusses life without John, as well as the signs and risk factors of aortic disease…

Seven years after actor John Ritter died, his widow, Amy Yasbeck, still thinks about him every day.

"I miss John and strive to keep his spirit alive," Yasbeck, 48, says.

In 2003, while rehearsing his TV sitcom "8 Simple Rules," Ritter felt severe nausea and chest pains. He was rushed to a nearby hospital, where doctors initially believed the comic actor, then 54, was having a heart attack.

In fact, he had suffered a thoracic aortic dissection – a tear in the thoracic aorta, the large artery that carries blood from the heart to the rest of the body. The aorta can rupture when the area where it exits the heart becomes enlarged, causing what's called an aortic aneurysm (swelling that weakens the walls of the blood vessel). Ritter died that night.

"Since John died, I've learned family history is the leading risk factor for thoracic aortic dissection," says Yasbeck. "I encouraged John's brother, Tom, to have his aorta scanned, and doctors discovered he had the same tear John had. Tom was successfully operated on and is doing well."

About 40,000-50,000 Americans die each year from aortic disease, according to the University of Texas Health Science Center at Houston, which has partnered with Yasbeck's John Ritter Foundation to educate doctors and patients about the disorder.

In this exclusive interview with Lifescript, Yasbeck talks about how her family's doing now and her "Ritter Rules" – simple tests that can help prevent others from dying as Ritter did.

What do you wish you and your family had known about aortic disease before John died?
In 1974, John's father, [country singer and actor] Tex Ritter, died at age 68 from an apparent heart attack. In retrospect, he probably died from an aortic aneurysm.

I want families to know aortic aneurysm risk factors and to get appropriate imaging [tests]. Our society has become heart-smart, but we're still aorta-ignorant in many ways.

Since John's death, have you seen progress in the field of aortic health?
Yes, just last year the American Heart Association and American College of Cardiology released new clinical guidelines, with recommendations for doctors on the diagnosis and management of thoracic aortic disease (TAD).

They deliver a powerful message to patients and physicians: Early diagnosis and treatment can save lives.

Besides genetics, what are some risk factors for thoracic aortic disease?
They can include poorly controlled high blood pressure, advancing age, smoking, male gender, atherosclerosis [a clogging or hardening of the arteries] and inflammatory diseases that damage the blood vessels.

If you have a family history of unexplained sudden death, talk to your doctor and see if you and your family members should be screened for TAD.

Is genetics a primary risk factor?
Dianna Milewicz, M.D., Ph.D, a professor and director of medical genetics at the University of Texas Health Science Center, has identified five genes that explain about 20% of cases of aortic aneurysms that run in families.

Members of more than 600 families, all of whom are related to someone who had an aortic aneurysm, have enrolled in a genetics study conducted by Milewicz as part of the John Ritter Research Program.

How are people screened for thoracic aortic disease?
Because the risk of aortic dissection increases with age, many doctors advise patients ages 65 and older [to get] screened with an MRI [magnetic resonance imaging] or CT [computerized axial tomography] scan that can detect an aneurysm before it tears.

If we can determine people predisposed with this disease, [they] can have their condition managed by their doctor through aortic imaging, genetic testing and blood-pressure monitoring.

How has John's death changed treatment of thoracic aortic disease?
John's death has led to more doctors considering TAD as a diagnosis in emergency rooms.

Because TAD symptoms mimic those of a heart attack, and since heart attacks are much more common (experienced by 920,000 patients annually), many patients with TAD symptoms are mistakenly diagnosed and treated for a heart attack – as John was. These treatments can exacerbate TAD.

Would John have been pleased about your work to promote awareness of thoracic aortic disease?
This is what John would have wanted. He always realized that with celebrity came great responsibility. I think if he had survived, he would have become an advocate for diagnosing and preventing TAD.

What do you hope readers learn from your memoir, With Love and Laughter, John Ritter (Gallery)?
The book's title has special meaning since those were the words John used when signing autographs. I hope readers will see the book as a celebration of his life.

What inspired you to write the book?
Our daughter, Stella, had turned 5 on the day John died, and there were so many memories I wanted to keep alive for her. I started writing them down on index cards, and I found that reading them aloud helped me too.

One of his favorite sayings was, "Here we go, here we go," and those were the words that pushed me to get out of bed in the days and months following his death.

Since Stella was so young when her father died, how did you explain his death to her?
I used a lot of John's philosophy on life. I tried to answer her questions honestly without overwhelming her with information.

It's a huge lesson for her to know that it's OK to be alive and go on living, and that it honors the person to live with a bit of their light inside you.

Seven years after John's death, how are you and Stella doing?
There are still good and bad days. In my book, I say that time doesn't heal wounds; it salts the wound.

John lives on through his three [other] kids, his brother and Stella. John's son Jason [actor Jason Ritter] and Stella both favor John in a lot of ways. Stella exudes his spirit – not just for comedy, but for the way she looks at life.

I've found the best way to honor his memory is to live the way he encouraged us to go through life. John always encouraged everyone to take time out of their day for random acts of silliness and goofiness.

Risk Factors and Symptoms of Aortic Disease -
With the help of Milewicz, who's also chair of cardiovascular research at the University of Texas Health Science Center at Houston, the John Ritter Foundation has created these "Ritter Rules" to help people recognize, treat and prevent aortic aneurysms and their complications. They are:

Rule 1 – Urgency
Thoracic aortic dissection is a medical emergency. The death rate increases 1% with every hour the diagnosis and surgical repair are delayed.

Rule 2 – Pain
Pain is the No. 1 symptom of thoracic aortic disease, Milewicz says. Seek immediate emergency medical care if you or a loved one experiences a sudden onset of severe pain in the chest, stomach, back or neck. The pain is likely to be sharp, tearing, ripping or "moving."

Rule 3 – Misdiagnosis
Heart attacks are far more common than aortic dissection, but if a heart attack or other diagnosis isn't clearly and quickly established, then aortic dissection should be quickly considered and ruled out, Milewicz says.

This is especially important if a patient has a family history of thoracic aortic aneurysm and/or dissection, or features of a genetic syndrome (see rule 6 below) that predisposes the patient to these conditions.

Rule 4 – Imaging
Only three imaging studies can identify aortic aneurysms and dissections: CT, MRI and transesophageal echocardiogram [a diagnostic ultrasound device that's inserted into the esophagus to create images of the heart muscle and other parts of the heart. A chest X-ray or EKG can't rule out aortic dissection.]

Rule 5 – Triggers
Lifestyle and trauma can trigger an aortic aneurysm and/or dissection, Milewicz says. It can happen through injury to the chest, extreme straining associated with bodybuilding, illicit drug abuse, poorly controlled high blood pressure, or by discontinuing necessary blood pressure medications.

In rare cases, pregnancy can trigger an aortic dissection. Women with aortic aneurysms and connective tissue disorders are at higher risk of aortic dissection during late pregnancy and delivery, and should be carefully monitored by a cardiovascular specialist.

Rule 6 – Risk Factors
Aortic dissections are often preceded by an enlargement of the first part of the aorta, where it comes out of the heart – called an aortic aneurysm. If you have an aneurysm, you're at increased risk for an aortic dissection.

A personal or family history of thoracic disease also puts you at risk, Milewicz adds. If family member is living with an aneurysm or has had an aortic dissection, you (and other family members) should be evaluated.

Certain genetic syndromes can also greatly increase your risk. They include:

•Marfan syndrome, a genetic condition that affects connective tissue. It also causes abnormalities in the cardiovascular system.

•Loeys-Dietz syndrome. Like Marfan syndrome, this inherited disorder affects connective tissue and the cardiovascular system.

•Vascular Ehlers-Danlos syndrome, another group of inherited disorders that make blood vessels more prone to rupture.

•Turner syndrome, a female chromosomal disorder that also causes abnormalities in the casdiovascular system.

•Bicuspid aortic valve disease (having two leaflets, which guard the heart opening, instead of the typical three). Also, if you've had a bicuspid aortic valve replaced, you need to be monitored for aortic disease.
Take Action:
  • Update: DADT has been repealed. President Obama has kept his promise!
  • Boycott Bill O'Reilly and FOX News and their advertisers.  Bill discussed a French commercial by McDonalds that was meant to show that McDonalds is Gay Friendly. Bill said what is next? Is McDonalds going to be Al-Qaida friendly as well?  Imagine - Bill O'Reilly compares Gays to Al-Qaida!
  • Boycott Arizona the home of  "hater" - U.S. Senator John McCain. Arizona's Hateful anti-Immigration Law - encourages racial profiling and increases hatred towards minorities.  A remedy: Demand Republicans to  support Immigration Reform (The Dream Act). Further, Senator McCain voted against ending DADT and Blocked Immigration reform!
  • Boycott Target, Best Buy, Gold's Gym - for donating money towards anti-gay political candidates/organizations. Update: (12/26/2010): Target is continuing to donate to anti-gay groups/causes/politicians. Update 03/08/11 - Lady Gaga Ends Target Partnership, Reportedly Over Target's Poor LGBT Stance. Update 3/25/2010 - Target displays their hypocrisy and Sues California Gay Rights Group for Lobbying Outside Stores.  Target is attempting to block the LGBT right of free speech!  STOP SHOPPING AT TARGET!!!!!!!!!!!!!!!!! Update 4/8/2011: Target lost it's case to stop the LGBT from canvassing in front of their stores.  A huge PR Disaster for TARGET!  Perhaps Target should go out of business as they betrayed their investers and customers. Target contributed massive funds to a politician who would like to exterminate gays.  Now, Target can not stop us from Boycotting them, nor picketing, nor talking to customers near their stores; although, Target allows a anti-gay company such as the Salvation Army to stand outside their door and speak with customers and raise money. Isn't this a double standard? Perhaps TARGET would better understand the issue if they cared about David Kato - Uganda, who was slain because he is gay! That is because Uganda wants to kill gay people.  Again, TARGET invested in a political candidate that would Kill the Gays!
  • End DOMA (Defense of Marriage Act).  Prevents Federal employees and Gays in the Military from gaining equal benefits. Prevents Gay Partners from gaining equal benefits.
  • Pass ENDA (Employee Non-Discrimination Act).  ENDA would prevent employment discrimination of  LGBT workforce.
  • Boycott Salvation Army as they will not hire Gays! "DO NOT DONATE TO THEM."
  • BOYCOTT "Chick-fil-a" for donating to anti-gay marriage groups.
  • BOYCOTT KOCH INDUSTRIES PRODUCTS- to include their products: Angel Soft toilet paper / Brawny paper towels / Dixie plates, bowls, napkins & cups / Mardi Gras napkins and towels / Quilted Northern toilet paper / Soft 'n Gentle toilet paper / Sparkle napkins / Vanity fair napkins / Zee napkins / Georgia-Pacific paper products & envelopes / All Georgia-Pacific lumber & building products (INVISTA Products) / Lycra / Stainmaster Carpet.   The billionaire - Koch brothers are ultra republican - tea bag founders/supporters that do not support causes of the LGBT community.
  • Boycott WALMART - The National Gay and Lesbian Task Force, force criticized Wal-Mart for denying employee benefits to same-s-x partners and for failing to prohibit discrimination based on gender identity. By comparison, the group said two other chain stores with a strong presence in New York, Costco and Walgreens, did much better.
  • NEW: Boycott Donald Trump [Tune Out TRUMP], The Apprentice and NBC Network! At CPAC, Trump said he is considering a run for president as a Republican. In a interview with the Des Moines Register, Trump added that he opposes all forms of legal recognition for gay couples, not just marriage. "They should not be able to marry," he said. So why does NBC keep the Apprentice when Donald Trump has stated he may run for president? Boycott NBC, CNBC! Boycott advertisers of The Apprentice and his other business's. Don't shop at any bullding with Donald Trump's name to include Trump Hotels, Casinos, Condo's, Offices, Shops!  The Donald has expressed extreme Hatred. He is a birther as well and bring out all the Bigets!  Remember how poorly Trump treated Rosie O'Donnel?  Now we know why he did that!  It was hate against a lesbian as we now know he is hater.  Unforgiveable!  Ready to dump NBC because they air Donald Trump's "The Apprentice"?  Start Watching CBS Evening News with Katie Couric (Real News that is Fair).
  • NEW: Boycott "APPLE." For the current owners and the aspiring owners of the I-phone,  Apple allows an application, "The Gay App", which is supposed to help gay individuals turn straight.  This is a step in the wrong direction.  Apple needs to know that the decision to allow this App on their products is a fallacious idea! Update: March 23, 2011:  Apple removed this app from USA stores; but, rumored to have them remain in some foreign stores.  This is offensive not just towards the LGBT community in the USA; but, offensive to  the entire LGBT community around the world.  How does anyone approve such a hateful, despicable app towards the LGBT community? Especially when Gay Teens are killing themselves after being harrassed by their church, parents, as well as  by bully's and teachers at school. The idea that this app was used, solicited is disgusting.  Still "no comment" from APPLE!

And Now, Model Wars!

This group is called "Peckers Pics."  The English -  slang definition of "pecker" is to pluck at the truth. Therefore, we peck at items such as Gay Men's Health, Male Fitness, Gay (LGBT) Politics & Issues.  In this section you may peck at each photo in order to decide the winner of the "war of the fittest!"   Whereas, who is the model that may inspire you to exercise and "get fit?"  Warning: This may stoke you!

Gay rights group: You're damn right we pressured law firm on DOMA
By Greg Sargent - Washington Post - Apr 26, 2011

Wow, so it looks like the right is trying to make a major issue out of the fact that leading gay rights advocates used some heavy-duty pressure tactics to get the law firm King and Spalding to drop their case defending the Defense of Marriage Act for House Republicans.

I just got off the phone with the Human Rights Campaign, the gay advocacy group that's in the right's crosshairs. The group's response, in a nutshell: Deal with it.

The latest round got started this morning, when the Weekly Standard published an internal email from the Human Rights Campaign detailing that HRC had "contacted many of the firm's clients" as part of its campaign to get King and Spalding to drop the case. Right wing bloggers, such as Jennifer Rubin, are pouncing on this as proof that the left engaged in an "unprincipled campaign" of intimidation to deprive the House of Representatives of legal representation.

Far from being abashed about this campaign, Fred Sainz, a spokesman for the Human Rights Campaign, shared new details about it. He confirmed to me that his group did indeed contact King and Spalding clients to let them know that the group viewed the firm's defense of DOMA as unacceptable.

Sainz said his group did not ask any of the firm's clients to drop the firm in retaliation for taking the case, as is being assumed by conservatives who are alleging an untoward pressure campaign. Rather, he said, his group informed the firm's clients that taking the case was out of sync with King and Spalding's commitment to diversity, which it proudly advertises on its Web site.

"King and Spalding's clients are listed on its web site, so we did what you would expect us to do," Sainz told me. "We are an advocacy firm that is dedicated to improving the lives of gays and lesbians. It is incumbent on us to launch a full-throated educational campaign so firsm know that these kinds of engagements will reflect on the way your clients and lawschool recruits think of your firm."

"We did all of this, and we're proud to have done it," added Sainz, who declined to name which King and Spalding clients his group contacted.

Conservatives are insisting that King and Spalding's decision to drop the DOMA case unfairly deprived the House GOP of legal representation in response to political pressure. But Sainz pushed back hard on that idea, pointing out that law firms are not obliged to take any case.

"We have a profound disagreement on the issue of whether or not law firms have the responsibility to take these kinds of civil cases," Sainz said. "We believe that no law firm has any responsibility to take a case. Firms pass every single day on cases they do not believe are consistent with their reputations. King and Spalding clearly agrees with that."

This issue is going to continue to heat up. More soon.
