AIDS-Free Survival Affected by CD4 Count at Start of cART

Combined antiretroviral therapy initiated at a threshold of 0.500 × 109 cells/L may increase the rate of AIDS-free survival compared to initiation at lower thresholds, but does not affect mortality, according to a study published in the April 19 issue of the Annals of Internal Medicine.

TUESDAY, April 19 (HealthDay News) -- Combined antiretroviral therapy (cART) initiated at a threshold of 0.500 × 109 cells/L may increase the rate of AIDS-free survival compared to initiation at lower thresholds, but does not affect mortality, according to a study published in the April 19 issue of the Annals of Internal Medicine.

Lauren E. Cain, Ph.D., from the Harvard School of Public Health in Boston, and colleagues evaluated the optimal CD4 cell count at which cART should be initiated. A total of 8,392 HIV-infected, therapy-naive individuals whose CD4 cell count decreased from at or above 0.500 × 109 cells/L at baseline to 0.200 to 0.499 × 109 cells/L, with no previous AIDS-defining illnesses, were included in the analysis. Hazard ratios and survival proportions for all-cause mortality were measured from the point at which the CD4 cell count dropped below 0.500 × 109 cells/L, and the study's combined end point was AIDS-defining illness or death.

The investigators found that the mortality hazard ratio was 1.01 for those initiating cART at a CD4 cell count threshold of 0.350 × 109 cells/L, and was 1.20 for those initiating at 0.200 × 109 cells/L, compared to the threshold of 0.500 × 109 cells/L. The corresponding hazard ratios for the end points of AIDS-defining illness or death were 1.38 for a threshold of 0.350 × 109 cells/L and 1.90 for a threshold of 0.200 × 109 cells/L.

"Delaying initiation until the CD4 cell count decreased below 0.350 × 109 cells/L did not seem to substantially affect overall mortality. The benefits of early initiation cannot be fully realized if most persons present for medical care with CD4 cell counts below 0.500 × 109 cells/L; thus, early HIV testing would be beneficial," the authors write.

  • Boycott Arizona the home of  "hater" - U.S. Senator John McCain. Arizona's Hateful anti-Immigration Law - encourages racial profiling and increases hatred towards minorities.  A remedy: Demand Republicans to  support Immigration Reform (The Dream Act). Further, Senator McCain voted against ending DADT and Blocked Immigration reform! 
  • Boycott Target, Best Buy, Gold's Gym - for donating money towards anti-gay political candidates/organizations. Update: (12/26/2010): Target is continuing to donate to anti-gay groups/causes/politicians.  Update 03/08/11 - Lady Gaga Ends Target Partnership, Reportedly Over Target's Poor LGBT Stance. Update 3/25/2010 - Target displays their hypocrisy and Sues California Gay Rights Group for Lobbying Outside Stores.  Target is attempting to block the LGBT right of free speech!  STOP SHOPPING AT TARGET!!!!!!!!!!!!!!!!!  Update 4/8/2011: Target lost it's case to stop the LGBT from canvassing in front of their stores.  A huge PR Disaster for TARGET!  Perhaps Target should go out of business as they betrayed their investers and customer base. Target contributed massive funds to a politician who would like to exterminate gays.  Now, Target can not stop us from Boycotting them, nor picketing, nor talking to customers near their stores; although,  Target allows a anti-gay company such as the Salvation Army to stand outside their door and speak with customers and raise money. Isn't this a double standard?  
  • End DOMA (Defense of Marriage Act).  Prevents Federal employees and Gays in the Military from gaining equal benefits. Prevents Gay Partners from gaining equal benefits.    
  • Pass ENDA (Employee Non-Discrimination Act).  ENDA would prevent employment discrimination of  LGBT workforce.
  • Boycott Salvation Army as they will not hire Gays! "DO NOT DONATE TO THEM."
  • BOYCOTT "Chick-fil-a" for donating to anti-gay marriage groups.
  • BOYCOTT KOCH INDUSTRIES PRODUCTS- to include their products: Angel Soft toilet paper / Brawny paper towels / Dixie plates, bowls, napkins & cups / Mardi Gras napkins and towels / Quilted Northern toilet paper / Soft 'n Gentle toilet paper / Sparkle napkins / Vanity fair napkins / Zee napkins / Georgia-Pacific paper products & envelopes / All Georgia-Pacific lumber & building products (INVISTA Products) / Lycra / Stainmaster Carpet.   The billionaire - Koch brothers are ultra republican - tea bag founders/supporters that do not support causes of the LGBT community. 
  • Boycott WALMART - The National Gay and Lesbian Task Force, force criticized Wal-Mart for denying employee benefits to same-s-x partners and for failing to prohibit discrimination based on gender identity. By comparison, the group said two other chain stores with a strong presence in New York, Costco and Walgreens, did much better.
  • Remember David Kato, Gay activist - Uganda, who was slain because he is gay!
  • NEW: Boycott Donald Trump, The Apprentice and NBC Network! At CPAC, Trump said he is considering a run for president as a Republican. In a interview with the Des Moines Register, Trump added that he opposes all forms of legal recognition for gay couples, not just marriage. "They should not be able to marry," he said. So why does NBC keep the Apprentice when Donald Trump has stated he may run for president? Boycott NBC, CNBC! Boycott advertisers of The Apprentice and his other business's. Don't shop at any bullding with Donald Trump's name to include Trump Hotels, Casinos, Condo's, Offices, Shops!  The Donald has expressed extreme Hatred towards gays and now blacks with his birther remarks and anti-fay rhetoric.  Remember how poorly Trump  treated Rosie O'Donnel?  Now we know why he did that!  It was hate against a lesbian as we now know he is hater.  Unforgiveable!  Dump NBC because they air Donald Trump's "The Apprentice"?  Start Watching CBS Evening News with Katie Couric.
  • NEW: Boycott "APPLE." For the current owners and the aspiring owners of the I-phone,  Apple allows an application, "The Gay App", which is supposed to help gay individuals turn straight.  This is a step in the wrong direction.
  • Apple needs to know that the decision to allow this App on their products is a fallacious idea! Update: March 23, 2011:  Apple removed this app from USA stores; but, rumored to have them remain in some foreign stores.  This is offensive not just towards the LGBT community in the USA; but, offensive to  the entire LGBT community around the world.  How does anyone approve such a hateful, despicable app towards the LGBT community? Especially when Gay Teens are killing themselves after being harrassed by their church, parents, as well as  by bully's and teachers at school. The idea that this app was used, solicited is disgusting.

Republican House to pay up to $520 per hour to defend gay marriage ban

The Republican-led House has agreed to pay a law firm up to $520 per hour to defend the Defense of Marriage Act banning federal recognition of same sex marriage, according to a contract posted by Politico.

Some quick background: The Defense of Marriage Act, the 15-year-old federal law that defines marriage as being between one man and one woman, is currently facing a legal challenge in New York. Traditionally the federal government defends challenges to federal law, but not in this case - after defending it for two years, the Obama administration announced in February it would no longer defend the law because it believes it is unconstitutional.

That decision left House Republicans, who largely oppose same-sex marriage, without "any choice" but to step in and defend the law, in the words of House Speaker John Boehner. In a letter earlier this week, Boehner asked for Department of Justice funds to be diverted to the House to pay for the costs of the defense, though that is not likely to happen.

Asked last month if Department of Justice would save money by not defending DOMA, Attorney General Eric Holder told the House Appropriations committee, "I'm not sure we save any money, frankly."

"The people who would be defending the statute, were we to do that, are career employees of the Department of Justice, who will not be spending their time doing that; they will be spending their time doing other things," he said. "I'm not sure that I see any savings as a result of the decision that I announced with the president."

The contract shows that the House has agreed to pay the firm King & Spalding up to $500,000 for its services in defending DOMA. It also says that Paul Clement, a partner at the firm, "will be principally responsible for conducting the Litigation."

As Josh Gerstein notes, Clement was Solicitor General under President George W. Bush.

House Minority Leader Nancy Pelosi sent Boehner a letter late Monday saying that "The House of Representatives need not enter into this lengthy and costly litigation." After the contract was revealed, Pelosi spokesman Drew Hammill cast House Republicans as wasting money despite their claims of fiscal responsibility.

"The hypocrisy of this legal boondoggle is mind-blowing," he said. "Speaker Boehner is spending half a million dollars of taxpayer money to defend discrimination. If Republicans were really interested in cutting spending, this should be at the top of the list."

Responded Boehner Spokesman Michael Steel: "Obviously, this whole thing would be unnecessary if the White House and the Justice Department would do their job and defend a law that was passed by both Houses of Congress and signed by the President of the United States - a Democratic President, at that."
