First Test of New Cancer Drug in People With HIV Is Promising

A newer cancer drug, Sutent (sunitinib), is tolerable when paired with at least some antiretroviral (ARV) drugs in people with HIV. The study results, presented at the 2011 meeting of the American Society of Clinical Oncology, being held June 3 to 7 in Chicago, represent an important step forward, as many cancer drugs are not tested in people with HIV.

Though rates of non-HIV-related cancers are on the rise in HIV-positive people, the science isn't keeping up with this new threat. Health care providers and their HIV-positive patients who have cancer must frequently make an extremely difficult decision--stick with known, but older and less effective cancer treatments, or risk serious side effects and drug interactions with newer treatments that have never been tested in people with HIV. This is because all studies of new cancer drugs currently exclude people with HIV from their trials, and the makers of those new drugs don't generally sponsor HIV studies at later time points.

In an effort to begin addressing this problem, John Deeken, MD, from the Georgetown Lombardi Comprehensive Cancer Center in Washington, DC, and his colleagues, designed a study of Sutent--which is approved to treat kidney and gastrointestinal cancer and which has the potential for serious drug interactions with some ARV drugs.

Deeken's group treated 19 people with HIV and cancer between 2008 and 2011. All of the people were on a stable ARV regimen and completed at least one cycle of Sutent therapy at 50 milligrams per day. Sutent requires the same liver enzyme, CYP3A4, to be broken down and eliminated from the body as a number of HIV drugs--notably Norvir (ritonavir). Some of the study participants were taking ARV regimens including Norvir, and some were not.

Deeken and his team found that people on regimens without Norvir did just as well on Sutent as HIV-negative cancer patients. They were no more likely to have side effects or more severe side effects. This group also had no ill effects on their immune systems. The group taking Norvir, however, did have additional side effects from Sutent, including neutropenia, which is a loss of white blood cells and which can be more of a problem in people with HIV.

The trial is not yet complete. This first round of data was focused on how or whether Sutent could be paired with ARV drugs. Longer-term safety and guidance on Sutent dose changes will be reported when the study is finished.

"Our HIV disease is now frequently being well controlled with HAART medications, but we are still having multiple medical problems including getting cancer earlier and more frequently," said James Weihe, an HIV-positive community representative for the AIDS Malignancy Consortium, one of the trial sponsors. "I am 60 years old and have been diagnosed with three minor cancers and two major cancers within the last two years.

"Dr. Deeken and the work his colleagues are doing give us new hope," Weihe continues. "Their research shows that we can be included in cancer research trials if the dosages of the medications are adjusted to avoid drug-drug interactions and other side effects."

PITTSBURGH -- As a gay person running for the City Council in Houston, which already has a lesbian mayor, Josh Verde figured he would have no trouble talking to voters about his sexuality. Then he came here, to a boot camp for openly gay candidates, and promptly flubbed his lines.

"I have a boyfriend," Mr. Verde announced during a mock interview with a campaign consultant playing the role of reporter. Instantly, he regretted the words. "It sounded like high school," he said later, amending his language to say, "I'm in a relationship."

Mr. Verde, a 31-year-old law student and aviation consultant, was one of roughly three dozen openly gay aspiring politicians who spent last weekend in a hotel conference room here for a crash course in campaign strategy. Run by the Gay and Lesbian Victory Fund, which works to elect gay men and lesbians from both parties to public office, the three-day session offered stark evidence of how far gay politicians have come -- and how quickly.

Thirty-five years after Harvey Milk captured America's attention in San Francisco as an openly gay politician, drastic shifts in public opinion are prompting gay men, lesbians, bisexuals and transgender people to seek, and win, public office as never before. The trend has produced all sorts of questions for gay candidates like Mr. Verde who feel comfortable talking about their sexuality -- and want voters to feel comfortable with it, too.

Here in Pittsburgh, where trainees included one celebrity of sorts -- Daniel Hernandez, the 21-year-old Congressional intern who helped save the life of Representative Gabrielle Giffords when she was shot in Arizona - a team of strategists schooled the participants in the nuts and bolts of running for office, including developing a message and hiring staff.

They dealt with issues like the importance of knocking on doors ("One way to break down barriers is to get introduced," advised Joe Fuld, the strategist running the session), how much money they must raise (twice as much as straight candidates, Mr. Fuld said) and what kind of photographs to use in campaign pamphlets.

"It's always the family, the kids," one would-be candidate lamented. "If you're a gay person who's single, do you put a picture with your dog?"

That gay politicians are even having such discussions is significant in itself. Not so long ago, elected officials like Representative Barney Frank, Democrat of Massachusetts, were closeted; Mr. Frank did not come out until 1987, seven years after winning his House seat. Now there are four openly gay House members, and the talk is about breaking glass ceilings.

In Houston, Mayor Annise D. Parker, elected in 2009, is running for a second term. In New York, Christine C. Quinn, the speaker of the City Council, is on the short list of potential Democratic candidates for mayor. In Washington, Representative Tammy Baldwin, a Wisconsin Democrat, is said to be considering a race for the Senate, a prospect that created buzz here among those eager for the first openly gay senator to be elected.

Modeled on Emily's List, the political action committee that works to elect Democratic women, the fund, with a budget of more than $5 million culled largely from gay donors, is a powerful force behind this trend, raising money, providing strategic advice and endorsing candidates like Ms. Baldwin, one of its rising stars.

In 1991, the year the fund was founded, it endorsed two candidates. Last year, it endorsed 164. The fund also helps closeted officials come out, and, through its Gay and Lesbian Leadership Institute, runs boot camps like the one in Pittsburgh several times a year.

The idea, said Chuck Wolfe, the fund's president and chief executive, is to build a "back bench" of politicians who can win at the local level and work their way up. Across the country, only five statewide officeholders are openly gay. Mr. Wolfe says he believes voters would accept a gay candidate for president, though he expects it will take 20 more years to produce a credible candidate.

"It could be a progressive, it could be a conservative," he said. "I don't have any predictions on the demographics of who it might be."

The Pittsburgh trainees came from across the political spectrum, from Marisa Richmond, a 52-year-old transgender historian who sits on the county Democratic Party's executive committee in Nashville, to Seth Kaufer, a 31-year-old gastroenterologist running for Republican ward leader in Philadelphia. ("I think it's an asset," he said of being a gay Republican. "I like to break stereotypes.")

Most, though, were Democrats, and most were young. Mr. Hernandez, now a student at the University of Arizona, is eyeing a school board seat.

John Campbell, 23, is about to become the new city treasurer of Harrisburg, Pa.; he raised $10,000 to beat a 58-year-old opponent in the Democratic primary last month, and is running unopposed in the general election.

Mr. Campbell said the training -- particularly Mr. Verde's flub -- resonated with his own experience on the campaign trail, where he found himself making subtle changes to his speaking style and appearance.

He ditched his form-fitting Guess jeans for looser ones to avoid looking like he was hitting the clubs. And he "made a very conscious effort" to use the word partner in describing his companion. "People saw that as something more valid than 'boyfriend,' " he said.

Sixty-two percent of Americans now say a candidate's sexual orientation does not matter to them, up from 51 percent four years ago, according to a survey published last week by the Pew Research Center. Of the estimated 500,000 elected officials in the United States, 495 are openly gay, up from 49 about 20 years ago, according to the Victory Fund.

Despite the increasing public acceptance, obstacles remain. Bruce Kraus, a gay member of the Pittsburgh City Council, told the trainees about the bricks thrown through his campaign office window a couple of years ago. Offensive language was spray-painted on some of his signs, and antigay literature was passed out at Catholic churches, Mr. Kraus said. Still, he said, "it's gotten infinitely better, and the proof in the pudding is I'm in public office."

Mr. Fuld, whose company, the Campaign Workshop, often advises gay candidates, said his clients frequently describe running for office as "coming out all over again." He gave trainees a list of 13 questions they should be prepared to answer, including, "How long have you been gay?" and "How long has your family known?" When one candidate asked how to describe his sexuality in campaign literature, Mr. Fuld pointed to Mayor Parker in Houston.

"She put on her literature that she was the LGBT police liaison," he said, using shorthand for lesbian, gay, bisexual and transgender. "You need to put it out there in some way, but it doesn't have to be the 'Joe the Gay Candidate.'"
 
Target chairman's 'apologetic' statement not enough for Colorado LGBT Consumers

Retail giant Target told investors this week that the company would not be choosing a side on the anti-gay-marriage amendment slated for the 2012 ballot in Minnesota. This decision was made as the Twin Cities LGBT Pride festivities get underway this weekend; with Target as a significant sponsor of the event. The announcement has sparked controversy across the country as to whether Target is offering a sincere mea culpa to the LGBT community, or is a well-times public relations effort in a month full of gay pride celebrations across the country.
 
"Our position at this time is that we are going to be neutral on that particular issue, as we would be on other social issues that have polarizing points of view," company CEO Greg Steinhafel told shareholders at the meeting held Wednesday. This was the first shareholder meeting held since the nationwide boycott of the retail chain was launched last fall in the wake of the corporations contributions to the campaign of then-Republican gubernatorial candidate Tom Emmer. Emmer lost the election in November to Democrat Mark Dayton.
 
The boycott itself had little effect on the fourth quarter earnings of the company, reporting net earnings of $1,035 million for the quarter ended January 29, 2011, compared with $936 million in the quarter ended January 30, 2010. Still here in Colorado, the question on the mind of the LGBT community is simple; is Target homophobic?
 
"I haven't shopped at a Target since the boycott began," says a Colorado gay man who asked not to be identified. "I question the policies of a company that tries to portray itself as a hip, trendy retail store. Their feeling about gays doesn't fit into my ideas of what's trendy."
 
Advocacy Chair of The Delta Foundation Thomas Waters, a Pennsylvania based LGBT group, attended the meeting, and offered his opinion; that it is incorrect to portray Target as homophobic.
 
"Target got painted as being 'homophobic,' which isn't really accurate, says Waters. "They acted in a way that was not aligned with their corporate values. That doesn't make them homophobes, but it does call into question their value system."
 
Target Chairman Steinhafel spoke in a similar vein when addressing the question.
 
"We have a broad team-member base, every shape and every size and every color. And so we are a very inclusive organization."
 
Still, Steinhafel's statements did draw a challenge from nearly a dozen shareholders about the company's choices when it came to political giving.
 
The issue stems from Target's choice to give a $150,000 contribution to MN Forward; and independent expenditure committee who produced a series of advertisements in support of candidate Emmer during the Minnesota campaign season last fall. According to the web site for MN Forward, their mission is "to elect a governor and legislators who understand the importance of creating private-sector jobs and economic opportunity in our state." The web site has been updated since the 2010 fall elections.
 
Target has vowed to "continue to monitor and assess," and see how their corporate policies shift. Target has said they made some changes to its corporate expenditures in February. Not necessarily the correct answer, according to The Delta Foundation' Waters.
 
"Is their support for the LGBT community real? More likely, it is, but there is a problem in the way the corporation acts and makes decisions regarding how they will use profits."
 
Here in Colorado the popularity of the retailer seems relatively unaffected. The store location in Glendale is still a bustling retailer in spite of the boycott. Still, Target's choice to remain mum on the issue of gay marriage is a start, but for many consumers and shareholders alike, the question of corporate spending in political campaigns is the heart of the issue. The consensus seems to be that the company may have learned from their controversial mistakes, but may not have learned so much that it prevents them from repeating them in the future.
